Community Access Programs

Receive the supports you need to re-engage with your local community. Join our Community Access Group and you will be able to meet with like minded peers, attend local events and participate in local activities. Form friendships in a social and casual environment, experience new activities and places led by group members and increase your independence all while having fun.

Overnight Respite

Our In-home respite provides you with respite from your care recipient. This service is very beneficial for the aged and people living with dementia as it maintains a familiar environment that they are comfortable with.

Our In-home respite provides you with respite from your care recipient. This service is very beneficial for the aged and people living with dementia as it maintains a familiar environment that they are comfortable with. In providing you with respite we will develop a tailored care plan so that you can rest with ease knowing everything will be looked after.

We are also able to offer respite at The Cottage in Kingscliff and where available 24/7 respite.

Our respite services are flexible and will be tailored to your needs.

Living With Dementia Program

This group program is specifically designed for people living with early stage dementia and their support person. The program is delivered over a five week period with one two hour sessions per week or as a two day workshop.

Description:

This group program is specifically designed for people living with early stage dementia and their support person.

The program is delivered over a seven week period two hours a day, with some flexibility.

During the program, participants have an opportunity to:

  • Obtain information and have questions answered
  • Meet with others who are in a similar situation.
  • Share experiences and express feelings in a safe environment.
  • Focus on maintaining and enhancing skills and abilities.
  • Explore ways of managing now and into the future.

The following eligibility criteria must be met to join a group:

  • The person has a diagnosis of dementia or is in the process of obtaining a diagnosis
  • The person is aware of the changes they are experiencing and wishes to attend a group.

Individual In-home Wellness Support

Lifebridge also offers in home individual support with qualified staff working with you to reach your personal goal in your own environment. Lifebridge can also offer the flexibility of after-hours services, weekend support and short term respite for carers needing a break or to run errands.

If you would prefer individual support to maintain independence, wellness or community access, Lifebridge offers flexible supports with the provision of qualified workers or volunteers to support you in achieving your goals. Take A Break

Take a Break is social group for people who have a cognitive impairment diagnosis. Group members determine the locations they want to visit and the activities they want to do.

In Take a Break, you will join a friendly and welcoming group and will develop friendships with other members. Take a Break is also a great opportunity to stay connected with the local community, events and activities.

Our Community Access Groups are designed to offer you a variety of activities, venues and new connections. The Community Access Group is a social group to help you develop friendships and supports with other group members and the local community.

Description:

Our Community Access Groups are designed to offer you a variety of activities, venues and new connections. The Community Access Group is a social group to help you develop friendships and supports with other group members and the local community.

Each member in the group is encouraged to express their ideas and suggestions as to group activities and locations to visit. The program is flexible so that you can do the things that you and other members want to do and achieve.

On offer are men’s groups, ladies groups and mixed groups. Enjoy the company of other members, have fun participating in various activities and broaden your community knowledge by visiting new locations and venues.
