What is a Home Care Package?

The Home Care Package program is an Australian Government initiative to help you live independently for longer. The program provides a subsidy towards a package of care, services and case management to meet your needs. Home Care Packages provide a greater level of support and enables you to create a tailored offering that will work for you. You choose when, where and how you need support. Our Home Care services can be provided privately or through Government funded support.

With a Home Care Package, your are in control. You have choice and flexibility in the way your care and services are provided and you agree to the level of involvement your want in managing your care package.

Levels of Service

There are four levels of support available. Each level of support offers a different amount of funding and is based on the level of care you need.

Level 1 Basic care
Level 2 Low-level care
Level 3 Intermediate level care
Level 4 High-level care  

Your level of support will determine the number of hours of care and services able to be delivered.

If you’re over 65 (or 50 if you identify as having Aboriginal or Torres Strait Islander heritage) and think you need some extra support with daily tasks, you can apply for a Home Care Package via the My Aged Care website. Our friendly staff can also guide you through the process of applying for funding.

How do I receive a Home Care Package?

Our 6-Step easy to follow process helps you to navigate through the necessary actions to receive a package.

If you’re not already registered, you will need to Register your details with MyAgedCare. They will do a phone screening to assess your needs and the care you’re currently receiving. Following this, they will then refer you for an Assessment by Aged Care Assessment Team.

After your assessment by ACAT you will receive a letter informing you of your eligibility status.

Respond to the letter via the number listed confirming your acceptance, when the package becomes available.

Once available, you will receive a letter with your Unique Identifier Number.

Once you have your letter, phone Lifebridge. Have your Unique Identifier Number available.

Lifebridge will arrange an appointment to commence discussions on your service requirements.
